The actor’s recent public appearances have been scarce, with his last known engagement being The Wizard of Oz premiere in Las Vegas in August 2025. A Quest for Answers Beyond Conventional Medicine
According to information released about the book, Manganiello will recount a previously undisclosed health ordeal. This period was marked by multiple life-threatening medical crises that propelled him on an extensive, global search for answers. His quest led him to explore his ancestry, inherited trauma, and the profound historical legacies that shape us.
The 49-year-old actor revealed that his health deteriorated rapidly and unexpectedly. He described a “cascade of autoimmune-related illnesses” that affected his skin, thyroid, eyes, lungs, and digestive system. He endured constant chronic pain, a life-saving organ amputation, and an existential crisis, all while doctors struggled to provide a clear diagnosis or explanation for his prolonged fight for survival.
Ancestry, Spirituality, and Resilience
In his search for understanding, Manganiello reportedly encountered a wide range of practices and beliefs, including shamanism, pagan rituals, ancient myths, and extensive genealogical research. His exploration into his family history uncovered the story of his ancestors who survived the Armenian genocide.
The memoir is also set to explore themes of inheritance, masculinity, faith, ambition, and the identities individuals construct to navigate adversity. Manganiello expressed his hope that sharing his experience in Bloodlines will offer solace and encouragement to others facing similar health challenges.
“It was the most brutally difficult time of my life, one I wouldn’t wish on my worst enemy, but also my greatest adventure,” Manganiello shared. “I hope that what I went through on this journey can give readers hope that answers and healing may lie for them on the other side of whatever they are fighting through. The process of writing this book gave me the gift of perspective that helped me to see that my suffering was a cocoon from which I would emerge forever changed.”

Beyond his health struggles, the past seven years have seen significant personal changes for Manganiello. He divorced actress Sofia Vergara in 2023, with the finalization occurring a year later. The former couple released a joint statement at the time, requesting privacy as they navigated their new chapter.
Manganiello has since become engaged to Caitlin O’Connor. The couple has relocated from Los Angeles to Pittsburgh. O’Connor commented on the move, stating, “LA has been both of our homes for over 20 years. I was born in LA, but raised in Pittsburgh, so we’re both from the same hometown.” She added, “That’s what we really connect on, we share the same upbringing, the same hometown knowledge. And that’s what really connects us as a couple. So it’s beautiful to be back home together.”
