A little over a year into his second presidential term, President Donald Trump confronts growing challenges as support from his core followers wanes. Psychology experts raise alarms about potential escalations in behavior now that the leader faces increasing scrutiny and isolation.

Resurfacing Epstein Allegations

Recent releases of documents related to Jeffrey Epstein include a 2016 complaint from an anonymous accuser using the pseudonym Jane Doe. The filing alleges that Trump raped a 13-year-old girl in 1994, describing multiple instances of abuse. It also claims Epstein expressed anger over Trump taking the accuser’s virginity and that Epstein himself assaulted her. These details align with the withdrawn 2016 lawsuit, filed just before the election that year.

Trump has denied these allegations. In 2016, Alan Garten, a lawyer for the Trump Organization, described the claims as a complete fabrication. The Department of Justice noted in a press release that the released materials may contain fake or falsely submitted images, documents, or videos, as they include everything submitted to the FBI by the public in response to the Freedom of Information Act.

Growing Frustrations Among Supporters

Trump’s polarizing style, marked by divisive rhetoric and unfulfilled promises of economic prosperity, fuels discontent. High inflation and unmet expectations have intensified recently, compounded by outrage over incidents involving Immigration and Customs Enforcement (ICE) officers, including the death of a U.S. citizen.

Even in traditional strongholds like Iowa, signs of fracture emerge. At a recent rally, where Trump floated the idea of running again in 2028, attendees heckled him. He dismissed the critics as paid agitators. This shift from unwavering loyalty to open criticism marks a notable change for the 79-year-old leader.

Trump’s communication often features bold claims, repeated assertions about the 2020 election, and dismissive responses to journalists, particularly women. In 2024, over 200 mental health professionals signed a letter highlighting symptoms of severe personality disorder, including malignant narcissism, labeling him an existential threat to democracy. The letter cited repeated lying, impulsivity, and lack of remorse.

Expert Analysis on Potential Responses

Psychotherapist Mark Vahrmeyer, spokesperson for the UK Council for Psychotherapy, explains that public challenges to narcissistic individuals trigger deep shame, leading to narcissistic rage. “When a narcissistically structured person is challenged in public, the trigger isn’t ‘hurt pride’ in the everyday sense. It’s narcissistic wounding: a puncture in the image they need in order to feel solid inside,” Vahrmeyer states. “That lands as shame and humiliation, and for this personality style, those feelings can feel intolerable, even annihilating.”

He adds that dissent from the MAGA base feels like betrayal, prompting escalation through rhetoric or punitive actions, such as intensified immigration enforcement. However, external constraints like courts and Congress may force adaptations, though these would likely frame as victories rather than admissions of error.

Dr. Katie Barge, a psychologist, notes that losing loyalty registers as a profound threat. “From a psychological perspective, when a leader with strong narcissistic traits begins to lose control, admiration, or unquestioned loyalty, it’s often experienced as a profound personal threat rather than a political setback,” she says. “What we typically see when narcissistic figures are cornered is escalation rather than reflection.” This could manifest in blame-shifting, fear-mongering, or risky decisions to reassert dominance.

International Concerns and Recent Incidents

Recent decisions, including the arrest of Venezuelan President Nicolás Maduro and threats against Iran, heighten global tensions. At the World Economic Forum in Davos, Switzerland, Trump confused Iceland with Greenland—a territory he has previously expressed interest in acquiring—raising questions about his mental acuity.

Slovakia’s Prime Minister Robert Fico, a Trump ally, reportedly expressed shock over the U.S. president’s psychological state after a January 17 meeting at Mar-a-Lago. During an informal gathering in Brussels on January 22, Fico described Trump as coming across as “dangerous.” White House Deputy Press Secretary Anna Kelly dismissed the reports as fake news from anonymous diplomats, emphasizing the meeting’s positive nature.

Trauma specialist and psychotherapist Tina Chummun warns of escalation over reflection. “From a clinical psychology perspective, when individuals with strong narcissistic traits experience public challenge, loss of admiration or threat to their status, what we often see is not reflection or repair but escalation,” she explains. “Narcissism is underpinned by a fragile sense of self and inner shame that relies heavily on external validation. When that validation is withdrawn, the response is commonly defensive rather than adaptive.”

In power, such responses amplify through authoritarian language and in-group narratives, prioritizing control over resolution.
